Surgical Management of Diabetic Traction Retinal Detachments

Vitrectomy is indicated for repair of diabetic tractional retinal detachments. Bevacizumab is being used off-label by some surgeons to decrease intraoperative bleeding during vitrectomy. However, bevacizumab injection may increase risk of macular traction, so it is important to perform vitrectomy within 1 week of injection. The so-called “chandelier” fiber optic intraocular surgical light made by Synergetics makes it easier to perform vitrectomy surgery. Laser treatment can be performed during the same procedure with an endophotocoagulator. Vitrectomy may be considered as an early intervention in eyes with very severe active PDR with the macula still attached.
