It has been noted that posterior vitreous detachment (PVD) can have beneficial effects in diabetic retinopathy. PVD can cause resolution of microaneurysms, involute nonproliferative diabetic retinopathy (NPDR), and reduce the risk of developing proliferative diabetic retinopathy (PDR) and tractional retinal detachment (TRD). Plasmin enzyme alters the composition of vitreous collagen. Plasmin and microplasmid induce PVD, promote liquefaction of the vitreous, and clean the retinal surface. Plasmin can also close macular holes. Plasmin is currently in European clinical trials as a treatment for NPDR with an Early Treatment of Diabetic Retinopathy Study severity grade ≤47.
